We know paperwork and assessments can be important for many different needs. While our providers aim to support you, some documentation can’t be completed because it requires in-person care or extended evaluation that telehealth can’t provide.
What Talkspace providers cannot complete
Talkspace providers can't complete, fill out, or sign assessments, or forms that require evaluation for eligibility, disability, accommodations, or legal/administrative purposes.
This includes but is not limited to:
- Family Medical Leave Act (FMLA) paperwork
- Leave of Absence (LOA) forms
- Social Security Disability (SS) evaluations
- Emotional Support Animal (ESA) letters or paperwork
- Hormone Replacement Therapy (HRT) evaluations
- Custodial or forensic evaluations
- Fitness for duty / return-to-work forms
- Housing, accommodation, or accessibility evaluations
These requests are outside the scope of our providers. We recommend reaching out to and working with your primary care physician or any other external physician who is able to complete formal assessments and required paperwork.
What you can request instead
If you need documentation about your treatment, your provider can prepare a treatment summary.
A treatment summary includes key information from your Counseling or Psychiatric Services Medical Record, such as:
- Assessments
- Treatment plans
- Progress notes
- Dates of service
This can be helpful when another clinician needs to understand your history, or when you need general documentation—but it can't replace formal assessment paperwork (FMLA, disability, ESA letters, etc.).
