What’s included in my medical records?
Medical records are comprehensive documents that include a member's health history, such as diagnoses, clinician notes, and all treatments received. If you request medical records for psychiatric services, they will also include psychiatric evaluations and medication history.
Components of medical records:
- Dates of Service: the date or date range of mental health service contact between a licensed provider and a Talkspace member.
- Diagnostic Assessments: a clinical evaluation designed to gather information on the member’s initial condition, current mental status, and the diagnostic impression.
- Individualized Treatment Plans: plans created for each type of service (e.g., counseling, psychiatric services) outlining goals and approaches.
- Progress Notes: details of the member’s symptoms, assessment, diagnosis, and treatment.
- Treatment Summary: a detailed overview of a member’s concerns, treatment received, and any side effects or other problems caused by treatment.
- Psychiatric Evaluations: a clinical evaluation that assesses a member's mental, social and psychological functioning.
- Medication History: a record of all medications that have been prescribed to the member through Talkspace.
Yes, you may request a treatment summary, a shorter, high-level overview of your care. In the records request form, just check off that you'd like "Other" information to be released and specify "treatment summary".
The treatment summary is prepared by your therapy or psychiatry Talkspace provider and includes:
- Your diagnosis
- Dates and duration of treatment
- Treatment goals and therapeutic approaches
- A brief description of progress
You might prefer a treatment summary if:
- You need general documentation and a full medical record would be more detail than necessary.
- You want a snapshot of your treatment for your personal files without requesting every note and assessment.
Most commonly, members request records for:
- Continuing care (e.g., sending a treatment summary to a new therapist)
- Insurance/clinical review (e.g., sharing progress notes or psychiatric evaluations with an external clinician)
- Personal records (e.g., maintaining a copy of your treatment history)

What are the steps to request my medical records or treatment summary?

Download the correct form (depends on who will receive the information):
- To request records for yourself: Individual Authorization for Release of Confidential Information
- To share your records with someone else: Third Party Authorization for Release of Confidential Information

Complete the form, reviewing the tips below to avoid processing delays:
- Make sure all writing and responses are clear and legible.
- Submit one form per recipient (i.e., one third-party form per external clinician).
- Make sure the name on your Talkspace account matches the name on the form.
- Specify exact dates whenever possible rather than “recent visits.”

Our timelines and follow-up guidelines
Our standard processing time for requests for medical records is up to 30 calendar days from receipt of a complete, signed request. If your request is incomplete, complex/extensive (e.g., for many years of care), or it was mailed, it may take longer than 30 days.
If you have an urgent request that needs to be expedited, please include the reason and the date you need the records in your initial submission. We'll try to prioritize urgent requests but cannot guarantee expedited turnaround.
To follow up on a request, please reply directly to the confirmation email sent by our Records team after your form is received. If you can't locate the email, you may contact the Records team at recordrequest@talkspace.com.
Kindly avoid emailing our Support team, as they are not able to assist with medical records—this is managed exclusively by the Records team.
