How Can I Get a Copy of My Medical Records? (Release of Information)

What are Medical Records?

Medical records are comprehensive documents that include a member's health history, such as diagnoses, clinician notes, and all treatments received.

Components of medical records:

  • Dates of Service: The date or date range of mental health service contact between a licensed provider and a Talkspace member.
  • Diagnostic Assessments: Clinical evaluations of a member's initial condition, current mental status, and diagnostic impression.
  • Individualized Treatment Plans: Personalized care plans outlining goals and approaches.
  • Progress Notes: Details of a member’s symptoms, assessment, diagnosis, and treatment.
  • Treatment Summary: An overview of a member's concerns, treatment received, and any side effects or issues related to treatment.
  • Psychiatric Evaluations: Clinical evaluations that assess a member's mental, social and psychological functioning.
  • Medication History: A record of all medications prescribed to a member through Talkspace.

How can I get a copy of my Medical Records?

To get a copy of your medical records, follow the process that applies to your age group:

For members age 18 and over

If you’re a Talkspace member who is 18 or older, you can request your medical records directly from your account by following these steps:

  1. Log in to your Talkspace account.
  2. Click Account (Account Icon.png) in the top-right corner of the screen.
  3. Scroll down and select Medical records.
  4. Select the service you need records for and click Continue.
  5. Select your preferences and click Continue:
    • Choose all records or specify a custom date range.
    • Choose the entire record or only the session dates and provider names.
  6. Enter your password and click Request records.
  7. You'll either be able to download your records immediately, or you'll receive a notice that your request has been sent to our Medical Records department. If your request is sent for processing, your records will be emailed to you within 30 days.
For members under age 18

If you are under 18, you won’t be able to access your medical records directly from your account. Instead, follow these steps:

  1. Download the correct form:
  2. Complete the form:
    • Ensure all writing is clear and legible.
    • Submit one form per recipient (i.e., one third-party form per external clinician).
    • Make sure the name on your Talkspace account matches the name on the form.
    • Specify exact dates whenever possible, rather than using “recent visits.”
    • Choose one or more of the following documents to release:
      • Counseling Services Medical Records
      • Psychiatric Services Medical Records
      • Other (e.g., specify if you want a "Treatment Summary" included)
    • Include a clear signature (wet or e-signature with timestamp).
  3. Submit the completed form using one of the following methods:
    • Email (recommended / fastest): recordrequest@talkspace.com 
    • Fax: 917-591-1861
    • Mail: PO Box 659, Portsmouth, NH 03802
      • Tip: If you choose to mail your completed form, consider sending it via tracked mail and allow for extra processing time.

Our standard processing time is up to 30 days after we receive your complete, signed request. Incomplete, complex, or mailed requests may take longer.
