How Can I Get a Copy of My Treatment Summary?

What is a Treatment Summary?

A treatment summary is a detailed overview of your care, including your main concerns, treatments received, and any side effects or issues related to treatment.

The treatment summary is prepared by your Talkspace therapy or psychiatry provider and includes:

  • Your diagnosis
  • Dates and duration of treatment
  • Treatment goals and therapeutic approaches
  • A brief description of your progress

You might prefer a treatment summary if:

  • You need general documentation and do not require all the details found in a full medical record.
  • You want a snapshot of your treatment for your personal files without requesting every note and assessment.

How do I get a copy of my Treatment Summary?

  1. Download the correct form:
  2. Complete the form:
    • Make sure all writing and responses are clear and legible.
    • Submit one form per recipient (i.e., one third-party form per external clinician).
    • Make sure the name on your Talkspace account matches the name on the form.
    • Specify exact dates whenever possible rather than “recent visits.”
    • Check off "Other" and specify "treatment summary" as the information to be released.
    • Include a clear signature (wet or e-signature with timestamp).
  3. Submit the completed form using one of the following methods:
    • Email (recommended / fastest): recordrequest@talkspace.com 
    • Fax: 917-591-1861
    • Mail: PO Box 659, Portsmouth, NH 03802
      • Tip: If you choose to mail your completed form, consider sending it via tracked mail and allow for extra processing time.

Our standard processing time is up to 30 days after we receive your complete, signed request. Incomplete, complex, or mailed requests may take longer.

To follow up on your request, reply to the confirmation email from the Records team. If you can’t find the email or have more questions, contact recordrequest@talkspace.com. Please do not submit these requests to the Support team.
