As a healthcare company, Talkspace must comply with HIPAA regulations to ensure the privacy and security of your medical information. To process your request for records, all submitted forms must meet these compliance requirements. We appreciate your cooperation and understanding as we work to fulfill your request.


Choosing the Right Documentation

Before requesting medical records, it’s important to understand what they include and whether they meet your needs.

Medical records generally include information such as dates of service, diagnostic assessments, individual treatment plans, progress notes, and treatment summaries. If you request medical records for psychiatric services, they will also include psychiatric evaluations and medication history.

Here's a brief description of each component of medical records:

    • Dates of Service: the date or range of dates of mental health service contact between a health service provider and a Talkspace member.
    • Diagnostic Assessments: a clinical evaluation provided by a licensed professional designed to gather information on the member’s initial condition, current mental status, and the diagnostic impression.
    • Individualized Treatment Plans: a treatment plan for each type of service provided (e.g., psychiatric services, counseling).
    • Progress Notes: includes details of the member’s symptoms, assessment, diagnosis, and treatment.
    • Treatment Summary: a detailed summary of a member’s concerns, the type of treatment received, and any side effects or other problems caused by treatment.
    • Psychiatric Evaluations: a clinical evaluation that assesses an individual’s mental, social and psychological functionality.
    • Medication History: a record of all medications that have been prescribed to the member through Talkspace.
NOTE: Instead of your entire medical records, you can choose to receive only a treatment summary. A treatment summary is a document prepared by a therapy or psychiatric provider that outlines the key aspects of a member’s treatment. It typically includes the member’s basic information, diagnosis, treatment goals, therapeutic approaches utilized throughout the sessions, and duration of treatment. The summary also highlights the member’s progress toward these goals and response to provider interventions.

Obtaining Medical Records or Treatment Summary

To obtain a copy of your medical records or treatment summary, fill out the attached Authorization for Release of Protected Health Information form in full. Email the completed form to recordrequest@talkspace.com or fax to 917-591-1861Please note that it can take up to thirty (30) days to process your request; keep in mind that a longer patient history may extend this timeframe. 

Tips on filling out the form to avoid processing delays

  • Submit one form per recipient, per submission.
  • Make sure all writing and responses are clear and legible.
  • Ensure the name on the Talkspace account matches the name shared on the form.
  • Provide either a “wet” signature (a handwritten signature on the physical copy of the form) or an electronic signature using software like Docusign that assigns a digital timestamp next to the signature.
  • Choose one or more of the following documents to release depending on the type of services (therapy counseling and/or psychiatry) received at Talkspace. For details on what medical records and treatment summary consist of, refer to the Choosing the Right Documentation section.
    • Counseling Services Medical Records
    • Psychiatric Services Medical Records 
    • Other (e.g., Treatment Summary for counseling and/or psychiatric services).
  • Specify the dates of services you'd like the medical records or treatment summary to cover.

Following up on your Previously Submitted Request

For updates on your previously submitted request, please reply directly to the confirmation email you received from our Records team when you first submitted your request. If you're unable to locate the email, you can contact the Records team at recordrequest@talkspace.com. Kindly avoid emailing our Support team, as they are not equipped to assist with medical records inquiries—this is managed exclusively by the Records team.


Talkspace Authorization for Release of Protected Health Information Form

Fill out the Authorization for Release of Protected Health Information form based on who you want to release the information to. If you are releasing it to yourself, use the Individual form. If you are releasing it to another party, use the Third Party version.
