When you first start Talkspace or add a new service, you'll be asked some basic questions about your health and medical history to help your provider get to know you. These questions make up your treatment intake assessment and allow us to customize your care.
To start on your intake assessment:
- Log in at www.talkspace.com or on the Talkspace app.
- Navigate to the Messages tab and select the room related to the service which you'd like to update your intake responses for.
- Scroll to the top of your room and find a button labeled "Treatment Intake".
- Click the button to open the form, which will appear in a pop-up window.
NOTE: If your intake assessment form does not load, please try our General Troubleshooting Tips to resolve the issue.
Once you submit your treatment intake questionnaire, you'll be ready to begin care with a provider.
Frequently Asked Questions
If you're having issues finding or opening the intake assessment link, including if it has expired or timed out, you can ask your provider to resend it.
If this is your first time messaging your provider, that’s okay—they’re happy to help. To send a message, open your room with your provider, start writing a message, and select Send.
If you have completed an intake form previously, you can send any missing or new information to be updated to your provider directly within your chat room. They'll be able to manually enter the information for you.
In the treatment intake, you may be asked questions about the following:
Parental/Guardian Consent (for Teen Members): If you're under 18, parental or guardian consent may be required to start services at Talkspace. Explore more at Teen Therapy at Talkspace: What Parents/Guardians and Teens Should Know.
Medication History (for Psychiatry Members): You'll be asked about any current or past medications, drug allergies, and your preferred pharmacy. This information helps your provider get a complete picture of your health and better inform your treatment plan.
Primary Care Provider Information: Providing your primary care doctor's contact information and your consent to share medical information with them supports coordination of your care. Sharing this information and/or your consent is optional. You may withdraw your consent at anytime by contacting privacy@talkspace.com.
Emergency Contact Information: Providing accurate emergency contact details helps us prioritize your safety. Provide your emergency contact's full name, phone number, and their relationship to you when requested.
NOTE: You may not be asked for all the information above if we already have it on file from a previously completed intake.
